Questionnaire
Most coversheet types require the investigator to complete a questionnaire, specific to the coversheet type, before the coversheet can be submitted.
If a questionnaire is required, a questionnaire tab will be visible next to the coversheet tab.
To find a coversheet previously entered:
- In the Human Ethics tab, select View Forms and click on Search. All draft and un-submitted coversheets will be listed.
- Click on the pencil button (edit)
- Click on the Questionnaire tab at the top of the page to complete the Questionnaire.
- The first question is displayed. After answering the question, click Next to view and answer the next question.

As you are answering the current questions, your answers to previous questions are shown.
Questions are presented one-by-one. Previously answered questions may be edited using the pencil button beside the answer.
The questionnaire has inbuilt logic to determine which questions are presented next, based on your previous answers. Therefore, any changes may result in some questions and draft answers being deleted.
As a result, we recommend that before changing any answer you create a copy of your draft answers first. See ‘Print a copy of your coversheet’.
The system will auto log-out after 20 minutes of being idle. If you are drafting answers, please ensure you hit ‘Next’ within 20 minutes of your last edit. This will ensure your draft content is saved – you can then edit further at a later time as required.
Questionnaire images and text based advice

If your change results in a different sequence of questions being applicable, you will be taken back to the relevant question to be answered next.
For this reason, if you click on the pencil button to edit a question, the following message is displayed:

After the Questionnaire is 100% complete, return to the Coversheet tab to review the Attachment checklist for documents required to be uploaded to the Documents tab.
Common control buttons and functionality
Move to the next question (the current question must be answered first, or an error message will display) | |
Deletes the last answer and presents the question before that with the answer removed | |
Open a new screen which displays the full questions and answers in the order they were presented (read only) | |
Restore a previously deleted answer to the current question. (This button is only displayed where an answer to the current question has been deleted) | |
