An incident report can be completed on behalf of another person, such as a staff member, student, visitor or contractor. Staff and students are encouraged to report their own incidents where appropriate. Reports submitted on behalf of a SCU staff or student will generate a confirmation email to the individual advising that a report has been submitted on their behalf and will enable their access to the report.

- In the 'Who sustained this incident?' section, select the Another Person option.

3. Use the icon to search and select the relevant staff member or student.
